醫院死亡已經越來越普遍,不僅成為治療疾病的地方,也是病人生命終點迎向死亡的地方,然而檢查、診斷、治療已成為現代治療主要目標,確實現在醫療為病患帶來「希望」,但當疾病已不可治癒可預期走向死亡時,生命品質卻不是醫院所關懷重點。現醫療管理者應重新尋獲醫院的價值主軸,應將轉換觀點,關懷人性價值的照護,而非將病患問題分割成各種專業化的處理,釐清真正的目標應重視「人的存在」與「人性價值」。將價值放在整個脈絡結構看待,如果不能提供價值的照護,醫院便只成為展現醫療科技進步的場所,而非病患與家屬身心療癒地方。 台灣人有46%都死在醫院,醫院必然扮演著不是對抗疾病的角色,並且是人性價值關懷及心靈療癒者的角色。或許對於護理人員、管理者而言這角色太過沉重,但事實證明,刻意忽視這角色,會造成更大的負擔與擔心、負面價值衝突,最後是管理者上成本更大的耗損。醫院管理者如能以「創造顧客價值」與「重視知識工作者價值感」,經營策略就愈能施展開來,將病患與家屬視為最重要的「顧客」,以增加服務品質與創造病人價值為目標;護理人員為醫療院所最重要的「資產」,重新去正視死亡療護管理過程,將死亡納入醫療中去面對,相信更能為醫療院所在永續經營的策略提出不同觀點。 Hospital is not only the place where disease is treated but also the common place where patients experience death. As the advancement of the medication, modern medical technique brings hope to the patients and their family. However, if the death could not be avoided, is cure still the emphasis we should pay on? Should we focus on the spirit and release the discomfort of the patients? Or the hospital is just the place where medical experts gather, not where patient can get comfortable treatment. Forty-six percent patients die in hospital. That is, most medical managers, including doctor and nurse, have to face the death of patient. It would be a task to deal with the problem well due to Chinese traditional concept. On the other hand, if we could give the patient and family a good course to face inevitable death, the quality of medical treatment will be more complete. Nurse played main role to care patients. If the manager could provide more education and resource about terminal care and death, the faculty of a nurse will be greater. A well-trained nurse, who can manage death and terminal course of indisposition well, must be a gift to patient, family and medical team.
