摘要:	隨著現代醫學的進步與生活品質的改善，智能障礙者得以延長壽命並有機會使用到社會福利資源，但近年來國內長期照顧政策的發展與規範，老年智能障礙者並未得到足夠的支持服務，面對其先天障礙的限制與身體機能退化而衍生的特殊照顧需求，為身心障礙教養機構在照護工作上帶來許多挑戰與困境。　　　本研究以某教養機構為例，透過資料檔案的回顧與整理、以及參與觀察和深度訪談等方式，解析老年智能障礙者家庭的照顧負荷與需求、以及身心障礙教養機構服務的供需現況和目前所面臨的服務困境。　　　本研究發現，老年智能障礙者家庭的照顧負荷相當多樣化，就質性訪談內容而言，結果顯現出家庭因為照顧工作普遍受到生、心理層面與社會層面的壓力，其中經濟負荷是主要影響家庭尋求老年智能障礙者照顧轉銜的因素。對於使用照顧服務資源的需求則以經濟安全、醫療與健康照顧、以及社會支持等三個層面為主要訴求重點。　　　從機構服務者的角度探究目前照顧服務供需落差的情形，針對研究分析結果發現，老年智能障礙者現階段的服務需求以生活照顧、健康與復健醫療、休閒娛樂活動的參與為主，但因受限於現行的法令政策之規範，使得老年智能障礙者在就養與就醫需求的層面上無法得到滿足與福利資源的支持，而對服務提供者而言，因各自分立的福利法案與照顧服務體系，使得照顧轉銜服務無法落實；機構方面，則因缺乏老年智能障礙者照顧相關之醫療專業知能與人力的投入、以及財務資源不穩定的情形下，使得照顧服務工作未能有效執行；此外低參與度的家庭支持亦會影響照顧服務的提供。　　　整體而言，老年智能障礙者的養護問題需要政府關注，並與民間社福機構或團體共同合作，才能有效舒緩目前身心障礙教養機構所面臨的服務困境，本研究依據以上的結論，對政府與政策、機構實務工作者、以及家庭等三個層面提出建議以供參考。　　Modern medicine and higher quality of life extend people's life. Accordingly, the mentally retarded have more chance use the social welfare resources in their elderly stages. Unfortunately, they are not supported well by recent policy making and regulations of long-term care. The mentally retarded elderly have difficulties due to their innate disabilities and degenerate physical functions. Institutional care services need to cater to the challenge.　　　This study focuses on a specific social welfare organization and involves review of documents, participant observation, and in-depth interview on the mentally retarded elderly. The analysis has two sides incorporated: their family care burden, particular needs, as well as the demand-supply situation of the institutional care providers and the real difficulties.　　　A variety of family care burdens are identified. Qualitative interview results indicate triple factors of care burden (physical, psychological, and social pressures). Economic impact on the mentally retarded elderly families calls for assistances from institutional care providers, especially in three aspects-economic security, medical and health care, and social support.　　　The gap between demand and supply of care service are explored from the point of view of institutional care service provider. It is found that the mentally retarded elderly need daily and health/therapy care, as well as leisure participation mostly. However, the needs are not fully supported by current law policies and not satisfied in the allocation of social welfare resources. Service providers have a common voice that current welfare system in Taiwan has divided two systems under which to deliver service to people with disabilities and the elderly. The care transition service of mentally retarded elderly would not function well. Experts and staff shortages are found in the institutional provider. Unsecured financial conditions further impede the work. In addition, low participation by the family members has a certain impact on the institutional care.　　　Care for the mentally retarded elderly--as a whole set of issues-demands the attention of the government. It also calls for a social welfare alignment between the public and the private sectors so as to relief the difficulties faced by institutional care service providers. This research has based on the conclusion above and provided some suggestions for reference in every aspect of policies, practical workers and families.
