    Title: 肝癌病患與其家屬在診療過程中的心理歷程與因應策略之探討

    Abstract:   肝癌的發生率及死亡率之高,肝癌患者在早期階段無明顯異樣,肝癌是所有癌症轉移中最惡性的,因肝癌轉移後存活率短,所以關懷照顧及了解肝癌病患與家屬是不容怠慢的。本論文旨在探討肝癌病患與其家屬在診療過程中的心理歷程與因應策略。    本研究是以敘事研究深度訪談的方式進行,研究者為放射診斷科之醫護人員,研究對象來源是以來放射科做血管栓塞術的肝癌患者與其家屬為主。本研究取得肝癌患者與其家屬同意,填寫了訪談同意書後,對這組人員進行各六次的深度訪談,以訪談逐字稿為依據,來分析肝癌病患與其家屬在診療過程中的心理轉折。本研究所獲得的結論有:一、疾病互動過程中啟發的意義;二、對器官捐贈、換肝、安寧病房此三件關鍵事件的不同點;三、病患與家屬互為依賴關係;四、哭泣的釋放能讓人有新的能量;五、醫療制度面向;六、對自己身體的照顧。研究中發現出病患對疾病感到絕望時,常會意志消沉及拒吃食物,家屬的適時激勵,可激發出病患求生的意志感,讓病患有努力活下去的信念。    期望本研究對於社會大眾與醫護人員,能更深入了解肝癌病患與其家屬在疾病過程中的內心世界,能關懷照顧這一群人,讓他們在絕望中有一道曙光,能更有希望感。
      The incidence and mortality rates of hepatoma are high in Taiwan. The early stage of patients with hepatoma did not show warning symptom and sign. The metastasis of hepatoma reveal malignant potential and the survival rate is low. The care and understanding of patients with liver cancer and their family members need no reflect. This article deals with the inquiry of the psychological process and coping strategies during the processes of diagnosis and treatment.The study is carried out by the doctors and nurses in the department of diagnostic radiology. The study design is descriptive and interviewing with the patients and their family. Six interviews were done for the psychological process of the cancer patients and their family members after the informed consents were signed.    In conclusion, the findings are as follows:  (1) The inspiration and meaning of interaction between medical professionals and patients and their family.  (2) The difference of critical events toward organ donation, liver tranplantation and hospice ward.  (3) The dependent role on patient and family.  (4) Regeneration of mood after crying.  (5) The attitude toward current medical system.  (6) The care of their body.    The study found down in the dumps and unwilling to eat when the cancer patient felt disappointed at their disease and treatment. The family member's prompt encouragement will inspire the patient and regain the belief of living.    We hope to light up a hope under the dark inner mind of cancer patients and their family and let the medical professionals understand that.
