Abstract:	研究者源於對當前安寧資源體系的關注，聚焦於安寧居家照護的社區醫療服務，對安寧居家的主要工作者－安寧居家護理師，進行安寧居家落實歷程以及安寧居家護理師的角色探討之研究。進而探討，安寧居家照護落實社區醫療服務的相關條件與歷程、以及安寧居家護理師在安寧團隊與社區安寧照護中所扮演的橋樑角色。本研究採用質化研究。以嘉義地區附設有安寧居家療護服務的醫療單位，作為組織背景、社區醫療服務的單位；落實安寧居家照護工作者—安寧居家護理師為主要研究的對象。資料的蒐集採參與觀察與訪談的方式，以動態參與、實地觀察安寧居家護理師的工作服務內容/流程、觀察安寧居家護理師與病患和家屬之間的互動、與安寧團隊的合作關係。透過訪談，蒐集安寧居家護理師的工作經驗、資源配置、以及安寧居家護理師在安寧團隊中的運作情形等歷程與感受。　　Researchers from the current concern about the hospice resource system, focus on the hospice home care in community medical services, according to hospice home care main workers- hospice home care nurse, to carry the course of the implementation of hospice home care, and to explore the role of hospice home care division. Further explore the implementation of hospice home care services, community health-related conditions and progress, as well as hospice home care division in the role play with the palliative care team and the community care. This study used qualitative research. According to Chiayi area have hospice home care services medical units, as the organizational context, community medical services units; the implementation of hospice home care workers - hospice home nurse as the main object of study. Collect information found in the observation and interviews, with participate in dynamic, observation of hospice home care services division of the work / process, observe the interaction between the hospice home care nurse with patients and their families, and the interaction of cooperation with the hospice team. Through interviews, collection of the hospice home care nurse work experience, and the allocation of resources, as well as the hospice home care nurse work progress and feelings in the hospice team.
