    摘要:   背景及目的:紅斑性狼瘡(Systemic Lupus Erythematosus;SLE)是一種透過自體抗體和免疫複合物造成器官傷害的自體免疫疾病。臨床表現相當多樣性,侵犯多個器官系統,還會造成嚴重的併發症。患者除了生理不適外,也造成心理的改變,甚至影響生活品質。本研究目的是探討紅斑性狼瘡患者的中醫體質與生活品質之相關性。  材料及方法:本研究採橫斷式設計,以問卷調查方法收集資料。研究對象為南部某區域教學醫院免疫風濕科門診招募年滿20歲之紅斑性狼瘡患者。問卷內容包含個人基本資料及生活型態、紅斑性狼瘡疾病活動度(SLE Disease Activity Score;SLE-DAS)及臨床表徵、中醫體質量表(The Constitution in Chinese Medicine Questionnaire;CCMQ)及紅斑性狼瘡患者生活品質問卷(LupusQoL_Questionnaire)。所得資料以統計套裝軟體IBM SPSS for Windows 24.0版本,進行數據處理與分析,依研究目的與不同之變項屬性而進行描述性和推論性分析。  研究結果:本研究共收集紅斑性狼瘡患者351人,男性有34人(9.7%)、女性有317人(90.3%);年齡層分布以30–39歲(27.9%)居多;其中20–29歲即確診為紅斑性狼瘡者佔123人;身體質量指數(body mass index, BMI)屬正常範圍有52.4%;教育程度以高中為主111人;婚姻狀況已婚者有207人;職業以無工作者居多佔129人;其中有57.3%自覺目前健康狀況為普通。單變項羅吉斯迴歸分析結果發現,LupusQoL 8個構面(身體健康、情緒健康、身體心象、疼痛、未來規劃、疲憊、親密關係、給他人帶來負擔)之分數與平和質體質的勝算比成正相關;與其他8種偏頗體質(氣虛質、陽虛質、陰虛質、痰濕質、濕熱質、血瘀質、氣鬱質、特稟質)則呈現負相關,且達到統計上顯著差異。  結論:本研究結果不僅有助於了解紅斑性狼瘡患者之體質特性,評估並針對疾病症狀進行中西醫或輔助療法介入參考,改善患者生活品質並增進患者生活參與。
      Background and Purpose: Systemic lupus erythematosus (SLE) is an autoimmune disease that can invade various organs and tissues in the body repeatedly and chronically inflamed. The clinical manifestations are quite diverse, invading multiple organ systems, and can cause serious complications. In addition to physical discomfort, it also causes psychological changes and even affects the quality of life. The purpose of this study is to explore the correlation between TCM constitution and quality of life in patients with lupus erythematosus.  Materials and Methods: A cross-sectional survey was used. Data was collected from the outpatient clinic of the Allergy Immunology and Rheumatology Center of the Regional Teaching Hospital in Southern Taiwan. The questionnaire includes lifestyle, SLE disease activity score; (SLE-DAS)and clinical features, the Constitution in Chinese Medicine Questionnaire (CCMQ) and patients with lupus erythematosus Quality of Life scale (LupusQoL).   Results: In this study, a total of 351 patients with lupus erythematosus were collected, 34 males (9.7%) and 317 females (90.3%); the age distribution was mostly 30-39 years old (27.9%); Among them 123 people was diagnosed Lupus Erythematosus during 20-29 years old; the body mass index (BMI) was within the normal range for 52.4%; 57.3% of them felt that their current health status was normal. The univariate logistic regression analysis of the LupusQoL and CCMQ in patients with Lupus Erythematosus found that: LupusQoL has eight facets (physical health, emotional health, physical mind, pain, future The scores of planning, exhaustion, intimacy, and burden on others are positively significant correlated with the neutral constitutions and negatively significant correlated with the other eight unbalanced constitutions.   Conclusion: The results of this study not only help to understand the physical characteristics of patients with lupus erythematosus, evaluate and refer to the intervention symptoms of Chinese and Western medicine or adjuvant therapy for disease symptoms, improve the quality of life of patients and increase their participation in life.
