    Abstract:   根據國發會的統計資料顯示,2014至2025年將為我國高齡人口成長最快速期間,推計2018年將超過14%成為高齡(aged)社會,於2025年達20%成為超高齡(super-aged)社會。現今台灣亟待建立一套完整的「長期照顧制度」,讓政府挹注更多的資源與預算,儘速發展「照顧服務產業」以因應日益龐大的老年人口和需要。  2007年內政部頒佈「長期照顧十年計畫」而後施行。此後,行政院亦著手研擬長期照顧服務法與長期照顧保險法擬銜接「長期照顧十年計畫」。研究者採取政策網絡理論並以台中市為個案,藉此廓清現階段長期照顧計畫政策執行的現狀與問題。同時研究者也期望借鏡日本實施介護保險的經驗,思考台灣長照政策如何收他山之石之效。  本研究發現台中市長期照顧政策執行之網絡關係,依據Rhodes & Marsh (1992) 所主張,乃是基於權威、資金和財源、政治正當性、組織能力、資訊等資源為依賴基礎,所形成的一種聯盟或利益共同體。此一網絡接近於Rhodes所歸類的「政策社群」網絡形態。但政府長照政策經費預算不足,網絡成員的運作各有其問題。  研究者對台灣長照政策提出以下建議:一、積極加快長照雙法進度,落實推動長期照顧服務制度。二、輔導長照市場自由化,提供多種專業服務模式。三、加強宣導建立高齡友善社會,增進民眾長期照顧服務之認知。四、強化網絡間溝通協調機制,主動發現民眾需求及提供長照服務。五、改善長期照顧勞動環境,提升照顧品質。六、重視健康預防政策,節省失能服務經費支出。七、保障受照顧者及其家庭權益,規劃申訴機構及支持網絡。八、建立長照對象評估及判定給付機制,避免標準不一支出浮濫。
      The statistics of the National Development Council show that the elderly population will grow rapidly during the period from 2014 to 2025. It is estimated that the elderly population will be over 14% and turn Taiwan to an aged society in 2018, and then a super-aged society in 2025 (up to 20%). It is urgent for Taiwan government to establish a well functional “long-term care system”. Under the system, the government is able to inject much more resources and budget for the development of care services industry to meet the needs of the growing elderly population.  The Ministry of the Interior promulgated “the 10-Year Plan for Long-term Care” in 2007 and implemented thereafter. To follow the 10-year plan, the Executive Yuan had also set out to draft “the Long-term Care Services Act” and “the Long-term Care Insurance Act” to respond to the growing demand for long-term care services.   To clarify the status quo and problems of the long-term care system in Taiwan, the research is based on the theory of policy network and illustrated with the case in Taichung city. It is also expected to take lessons from the experience of Japan.  The study found that the policy network of the implementation of long-term care system in Taichung is close to “policy community” classified by Rhodes & Marsh (1992). It is an alliance or interest group exchanging resources including authority, capital, financial resources, political legitimacy, organizational skills and information. But the government’s budget in long-term care policy is insufficient, the members of the network have their specific problems.  The research made suggestions for Taiwan’s long-term care policy as follows. 1. Accelerate the legislation progress of “the Long-term Care Services Act” and “the Long-term Care Insurance Act” to implement the policy efficiently. 2. Supervise the market liberalization to provide diverse care services for the elderly. 3. Promote a senior citizens-friendly society and enhance the awareness of the masses about long-term care services. 4. Intensify the mechanism for communication between networks to meet the needs and provide services initiatively. 5. Improve the working environment of care workers and the quality of caring services. 6. Emphasize disability prevention policy and control the expenditure on caring services. 7. Attend to the rights of the elderly and family carers, setting up complaints institutions and support networks. 8. Establish a standard mechanism of assessment and payment to avoid the waste of resources.
